Whiterun:
Bow of Shadows: Speak to Proventus Avenicci. He'll give you a note.
Civil War Champions: Read "Battle of Champions" note at The Drunken Huntsman.
Daedric Plate Armor: Read "Death of a Crimson Dirk" in the Dragonsreach Dungeon.
Farming: Investigate Goldenhills Plantation east of Rorikstead. You'll want to bring something strong against undead enemies.
Orcish Plate Armor: Read "Guard's Dossier: Yakhtu gra-Orkulg" note in the Whiterun Guard Barracks. You can craft Orcish Plate Armor if you have the Orcish Smithing Perk. (Not sure if you have to find the armor set first to be able to craft it.)
Pets of Skyrim: Read "For Sale" note on the table at The Bannered Mare.
Silver Armor: Read "M'Sharra's Diary" note in The Bannered Mare. It's over by the fire pit where Saadia works. Note that you'll be ambushed at the end of this quest, so prepare accordingly. You can craft this armor set once you have the Advanced Armors perk. It can be found in the Misc. section of the smithing table.
Staff of Hasedoki: Read "Smuggler's Trade Notes" at the bandit camp west of Whiterun.
Tundra Homestead: Purchasable from Proventus Avenicci.
Falkreath:
Bittercup: Read "Mysterious Altar" journal on the nightstand in the bedroom at Dead Man's Drink. This quest has three paths to choose from, each with a different reward. Each path gives you the titular Bittercup, which permanently raises your highest stat (Magicka, Health, or Stamina) by twenty points, and permanently lowers your lowest stat by twenty points.
The Path of Fortune leads you to a huge treasure trove worth thousands of gold. You're tasked with stealing a letter seal from a very wealthy High Elf in Markarth. Having a high speech or pickpocket skill is advisable. If not, you can travel to Solstheim and poison the Sujamma, and then trick the High Elf into drinking it at the Silver-Blood Inn. Do not try to take on the High Elf or his Dark Elf bodyguard in a fight, because they're both very high level and really strong. Once you've stolen the High Elf's letter seal, go to the Markarth Warrens, equip the courier's clothes and then go tell the High Elf these two options in order: "I've been looking for you." and then "Got something I'm supposed to deliver - your hands only." Once his bodyguard's been taken care of, follow the High Elf into the Markarth Warrens and let the rogue that sneaks in there with you deal with him. Once you're done with that, kill the rogue, and collect your treasure (He turns hostile if he detects you collecting it while he's still alive). Note that all the treasure here is marked as stolen, so make sure to consider that when you're looking to sell or for roleplay reasons. The treasure chests and the vault seem to have a high chance of spawning random enchanted items, so if you do this quest early, you could potentially get a lot of powerful enchantments for free. There's also an improved version of the transmute mineral ore spell that turns iron ore directly into gold ore.
The Path of Nothing takes you to an island populated by hostile ice wraiths, spriggans, and giants, and gives you a recruitable follower and their unique nordic dagger.
The Path of Power takes you to an arena with a ton of random high level weapons and armor (some of them potentially enchanted!), and a bunch of unavoidable fights with powerful enemies, one of which has a unique helmet and sword.
Note that you can only choose one path per playthrough, so choose... wisely.
Elven Hunter Armor: Read "Guard Dossier: Aesrael" note on the table in Falkreath Barracks.
Solitude:
Dead Man's Dread: Read "The Restless" book at The Winking Skeever.
Elite Crossbows : Read "Kragrash's Letter" note at Ironback Hideout.
Expanded Crossbow Pack : Fihada at the Fletcher sells these based on your level. Once you buy one, (aside from the Dwarven, Ebony, and Elven Crossbows) you can craft them yourself, provided you have the requisite smithing perks.
Lord's Mail : Read "Letter to General Tullius" note at Castle Dour.
(Enchanted) Steel Soldier Armor: Read "Suicide at Dragon Bridge" note in Four Shields Tavern.
Morthal:
Myrwatch: Read "Hans's Journal" note at a tower east of Morthal.
Dawnstar:
(Enchanted) Iron Plate Armor: Read "Nightgate Inn Patron's Note" in the Nightgate Inn. You can craft the regular Iron Plate Armor from the start of the game.
Saturalia Holiday Pack: Speak to Agrane Peryval west of Dawnstar.
Vigil Enforcer Armor Set: Read "Letter to Keeper Carcette" at the Hall of the Vigilant. If you're level 10 or higher, or have started Dawnguard, check under everything.
Markarth:
Divine Crusader Armor: Found in Four Skull Lookout. There's a light armor set and a heavy armor set.
Dwarven Armored Mudcrab: Purchasable from Calcelmo.
Dwarven Plate Armor: Read "Looter's Note" in the Silver-Blood Inn. You can craft Dwarven Plate Armor once you have the Dwarven Smithing perk.
Forgotten Seasons: Investigate Runoff Caverns west of Lost Valley Redoubt.
(Enchanted) Leather Scout Armor: Read "Guard's Dossier: Ehlhiel" note at Cliffside Retreat. You can craft the regular Leather Scout Armor from the start of the game.
Spell Knight Armor: Read "Crypt of the Heart - Draft" note at the Silver-Blood Inn. You get a different set of enchanted Ebony Spell Knight Armor depending on how you choose to resolve the "Crypt of the Heart" quest; one with offensive enchantments, or one with defensive enchantments.
Stendarr's Hammer: Stealable from the Dwemer Museum.
Riften:
Chrysamere: Found in Forelhost.
Dawnfang & Duskfang: Follow the ghost in the Riften Ratways.
Dwarven Mail Armor: Read "Arena Fan's Note" at the camp southeast of Ivarstead.
Goblins: Read "Letter to Clexius" note at The Bee and Barb.
Orcish Scaled Armor: Read "Guard's Dossier: Antonius" note in Mistveil Keep Barracks. You can craft Orcish Scaled Armor once you have the Orcish Smithing perk. (Not sure if you have to find the armor set first to craft it yet.)
Redguard Elite Armaments: Defeat Azadi in a brawl at Shor's Stone and read the note he gives you.
Shadowfoot Sanctum: Purchasable from Vekel the Man.
The Gray Cowl Returns!: Investigate the Riften graveyard.
Umbra: Read "Vigilant's Report" at Champion's Rest.
Windhelm:
Daedric Mail Armor: Speak to the Candlehearth Hall innkeeper. Note that to get the armor, you have to trade a very powerful illusion ring you get over the course of the quest. I actually recommend keeping the ring and just paying the 5000 gold for the armor or just ignoring that part of the quest entirely, because the ring is much more useful than the armor, in my opinion.
Dragonscale Armor: Read "The Crimson Dirks, v4" book in Candlehearth Hall.
Gallows Hall: Read "Naara's Journal" at the Abandoned Fort north of Mara's Eye Pond. This home apparently has an altar that can turn empty grand soul gems into black soul gems(!)
Goldbrand: Investigate the Sacellum of Boethia east of Windhelm.
Ruin's Edge: Investigate Stony Creek Cave.
Shadowrend: Investigate the hot springs near the Atronach Stone. (Note that you can switch between the Battleaxe and Greatsword form by checking the geyser where the weapon was while having it in your inventory.)
Sunder & Wraithguard: Read "Lost Caravan Guard's Note" at New Gnisis Cornerclub.
The Contest: Read "Adonato Leotelli's Journal" at Candlehearth Hall.
Winterhold:
Arcane Accessories: Purchasable from vendors or found in Hob's Fall Cave.
Arcane Archer Telekinesis Arrows: The spell is found on Urag gro-Shub's desk in The Arcanaeum. You'll still need to craft the telekinesis arrows to actually use the spell.
Arms of Chaos: Read "Hyenril's Journal" at Skytemple Ruins.
Nchuanthumz: Read "Seeks-Ancient-Artifacts' Journal, v1" at the Frozen Hearth.
Necromantic Grimoire: Can be purchased or found. Phinis Gestor sells the spells if your conjuration level is 65 or higher, robes and spell tomes can be found on boss level necromancers.
Wild Horses: Read "Soran's Journal" at the Arcaneum.
Solstheim:
Arcane Archer Soul Stealer Arrows: Found in the last chamber of Kagrumez. You can still rarely purchase these from general goods vendors before you complete Kagrumez, but you won't be able to craft them until you get the schematics.
Ghosts of the Tribunal: Read "Heretic Dossier: Blacksmith's Confessional" at the Raven Rock Temple.
Netch Leather Armor: Read "Peddler's Journal" on a Dunmer northwest of Skaal Village. This also gives you access to The Boots of Blinding Speed.
Nix-Hound: Purchaseable from The Retching Netch or from Revus Sarvani by the Silt Strider.
Staff of Sheogorath : Read "Mysterious Note" at the Retching Netch.
Stalhrim Fur Armor: Read "Skjol's Journal" at the camp southwest of Skaal Village.
Miscellaneous:
Arcane Archer Arrows: Every arrow not mentioned above can be crafted or bought from general goods vendors and hunters \ archery shops. They also occasionally drop on enemies. You can purchase the Bound Quiver from Calcelmo, Falion, and Phinis Gestor if your Conjuration skill is 45 or higher.
Bloodchill Manor: Read the letter from a courier at level 12. This quest is different if you're a vampire when you get the letter.
Bone Wolf: Read the letter from a courier after completing "The Wolf Queen Awakened".
Camping: You can craft camping supplies from the start of the game.
Dragon Plate Armor: Ask an innkeeper for rumors until a new dialogue option appears.
(Enchanted) Ebony Plate Armor: Read the letter from a courier at level 32.
Elven Horse Armor: Purchasable at any stable or blacksmith.
Fearsome Fists: You can purchase, find, or craft these. Note that the versions you craft don't come with any buffs to unarmed damage by default.
Fishing: Use any Fishing Supplies. The first one is next to The Guardian Stones, and another one is right before the bridge leading out of Riverwood.
Headman's Cleaver: Ask innkeepers if they've heard any rumors lately.
Hendraheim: Read the letter from a courier at level 10.
Nordic Jewelry: Purchase, find, or steal Nordic Jewelry and read the "Certificate of Authenticity".
Plague of the Dead: Read the letter from a courier at level 5.
Rare Curios: Purchasable from Khajiit Merchants.
Saints & Seducers: Speak to Ri'Saad at his Khajiit Caravan.
Staves: You can purchase or find these, and craft them if you have access to a Staff Enchanter.
Steel Horse Armor: Purchasable at any stable or blacksmith.
Survival Mode: You can turn this on or off in the settings menu once you leave Helgen.
The Cause: Read the letter from a courier at level 46.
