This Guide
In this guide I will go over the steps to get to the DLC area the quickest from New Game+. Base game spoilers ahead, but completely spoilerfree for the DLC.
This route is technically also viable when playing from a fresh game, but you're going to be dramatically underlevelled for Mohg's fight.
Step 1: Obtain A Great Rune
The Great Rune I recommend is Rennala, especially if you consider respeccing later. To get there easy, take the short route to skip Castle Stormveil. From there, head to the dragon in Liurnia of the Lakes and grab the Glintstone Key behind him (You don't need to defeat him.) From there on, go through any of the two main gates into the Raya Lucaria Academy. Just go through the dungeon as you would and defeat Rennala to obtain her Great Rune.
Step 2: Varré's Quest First Part
Next up is to set ourselves up for a shortcut to Mohg, Lord of Blood. First, go to the Roundtable Hold and talk to Enia near the Two Fingers. Next, go back to Liurnia of the Lakes and visit Varré who is now in front of the Rose Church. Tell him something is off and exhaust his dialogue. You can go online and do the invasion this way. For now, we move on and address the second part of his quest later, which is mandatory if you're playing offline.
(Also, if Varré is not available for whatever reason, you'll have to take the long way around to Moghwyn Palace, which requires the two Haligtree Medal pieces. I'm not going over that in this guide.)
Step 3: To The Altus Plateau
We should now aim to reach the Altus Plateau to unlock Radahn's fight and reach the offline portion of Varré's quest. I recommend taking the quick route via the Ruin-Strewn Precipice, but you can take the long way around and get the medallion pieces from Caelid if that floats your boat. Whichever way you take, you only need to activate a grace on the plateau to unlock the Radahn fight.
Step 4: Defeat Radahn
You can now travel to Caelid and go to the great bridge outside of Redmane Castle. The teleporter should be activated and you can just go into the castle. All you have to do is defeat Radahn, you don't have to go to Nokron or anything else.
Step 5: Varré's Quest Second Part
Now that you have access to the Altus Plateau, you can reach Writheblood Ruins and find a red invasion sign on the ground. This step is mandatory if you're playing offline, and optional if you can go online and invade someone else's world. Defeat Magnus the Beast Claw through his red invasion sign. After whichever step you take, return to Varré at Rose Church. Agree to his terms and services. You'll get the Lord of Blood's Favor.
Step 6: Varré's Quest Final Part
For the Lord of Blood's Favor you will need the blood of a maiden. You have a few options for the Maiden's Blood. You can kill Hyetta for it, possibly Irina if she's still there. If you have an Imbued Swordkey you can get it from the second Bellfry, which takes you to the Chapel of Anticipation. If none of those are options for whatever reason, I instead recommend going through the Frenzied Flame Village in Liurnia of the Lakes and going to the Church of Inhibition. You will have to be invaded by and defeat Vyke to get there. When that's done, return to Varré at Rose Church. Exhaust his dialogue. He will give you the Pureblood Knight's Medal. You can use it to go to Mohgwyn Palace.
Step 7: Defeat Mohg, Lord Of Blood
Now to use the Pureblood Knight's Medal and traverse through Mohgwyn Palace. Go through it and defeat Mohg, Lord of Blood. If you need it, you can go out of your way to get Mohg's Shackle and the Purifying Crystal Tear. After you defeat him, rest at the grace and Leda should appear next to Miquella's cocoon. Congratulations, you can now play the Shadow of the Erdtree DLC.
